This course examines how social determinants, including socioeconomic status, education, healthcare access, neighborhood conditions, and social support, can influence pain vulnerability and contribute to disparities in perioperative pain care. Learners will explore the roles of chronic stress, allostatic load, inflammation, and epigenetic aging in pain outcomes, while identifying practical strategies for equitable risk assessment, multimodal analgesia, regional anesthesia, and postoperative support.

Learning Outcomes:

  1. Describe the major social determinants of pain and specific disparities in regional anesthesia utilization across socioeconomic and racial/ethnic groups.
  2. Explain the biological mechanisms by which chronic stress and allostatic load contribute to pain vulnerability.
  3. Analyze the role of epigenetic modifications and DNA methylation-based aging clocks in mediating the relationship between social disadvantage and chronic pain.
  4. Apply evidence- based preoperative risk strategies that include social, psychosocial, and biological aging factors to identify patients at high risk for adverse pain outcomes.
  5. Evaluate clinical interventions and system-level approaches to reduce perioperative pain disparities, including equitable implementation of multimodal analgesia and regional anesthesia techniques.
