Sustained Implementation of an Evidence- Based Extubation Checklist Reduced Extubation Failure in Trauma Patients

A study published in the AANA Journal explored the use of an extubation checklist to reduce extubation failure in trauma patients. The study was conducted at a level I trauma center, where a five-item checklist was implemented for all patients who were electively extubated in the operating room. The extubation failure rates before and after the implementation of the checklist were compared. <br /><br />The study included a total of 26,867 trauma patients who were extubated in the operating room after the intervention. Out of these patients, 84 cases (0.31%) experienced extubation failure in the immediate postoperative period. A significant decrease in the rate of extubation failure was observed after the implementation of the checklist. Partial completion of the checklist, higher ASA physical status score, advanced age, and longer case length were found to be associated with increased odds of extubation failure in the post-intervention period. <br /><br />The authors concluded that the use of an evidence-based extubation checklist was associated with a sustained reduction in extubation failure in trauma patients. They suggested that integrating the checklist into the electronic health record and regular refresher workshops for anesthesia providers could further improve compliance and patient safety. Future research could include prospective studies comparing checklist utilization and evaluation of the checklist in different patient populations, including non-trauma patients and ICU settings. Overall, the study highlights the effectiveness of using checklists in reducing extubation failure rates.
