Lack of evaluation of the efficacy and safety of buprenorphine as an LA additive in prolonging the duration of analgesia.

Unrelieved postoperative pain remains a significant patient issue resulting in physical or psychological changes that leads to increased morbidity and mortality, increased costs, and a decrease in quality of life. The use of local anesthetics (LAs) in peripheral nerve blocks (PNBs) provides better analgesia than opioids alone; however, adequate pain relief in the early postoperative period is limited by the duration of action of the LA. While it is possible to place a catheter for postoperative pain management, catheter placement requires technical skills, additional time, and resources. Additionally, catheter placement may create unintended problems such as inaccurate catheter tip placement resulting in block failure, catheter knotting, an increased risk of infection, and neurologic complications. Hence, anesthesia providers seek to extend the benefits of single-shot PNB using LA additives.  

The anesthesia provider is equipped with the knowledge to use buprenorphine as an effective adjunct in extending analgesia and mitigating pain scores and opioid usage when appropriate.
