This presentation will discuss the current recommendations regarding preoperative management of GLP-RAs and the utility of gastric ultrasound in preventing the risk of pulmonary aspiration in patients taking these medications. GLP-1RAs are proving to be very effective medications for diabetic management as well as generalized weight loss which has increased its utilization in the diabetic and obese patient populations. Despite numerous benefits that these medications offer, significant gastrointestinal side effects have been noted including gastroparesis. There is consensus in the literature that patients taking GLP-1RAs who have followed current fasting guidelines are presenting to the operating room with full stomachs increasing the risk of pulmonary aspiration. Point-of-care gastric ultrasonography is a valid and feasible tool to assess gastric contents in diabetic and obese patients highlighting the need for anesthetists to become proficient in this skill.This is emphasized by the gap in the literature that exists regarding the appropriate amount of time in which GLP-1RAs should be held prior toelective surgery. Learning outcomes for the nurse anesthetist include understanding the current recommendations regarding preoperative management of GLP-1RAs and understanding the appropriate utilization of gastric ultrasonography when the gastric contents of a patient is inquestion.

  1. Discuss the current evidence-based recommendations regarding preoperative management of patients taking GLP-1RAs.
  2. Discuss the pillars of utility of point-of-care gastric ultrasound in patients whose gastric contents are in question.
